Register As A Patient
If you wish to register as a patient at our practice, please ask the receptionist for a new patient form. When you have completed and signed the form return it to our reception desk. You will be asked for proof of identification for our records and you will be allocated a new patient appointment.
If you prefer a particular doctor we will note this and do our best to respect your choice. However, not all doctors in the practice provide all services and specific doctors may not be immediately available.
Repeat Prescriptions –
You can order repeat prescriptions from 11am – 4pm by phone . You can collect the prescription 48 hours later, we will ask you a preferred local pharmacy at the time of ordering your medication and you will be able to collect your prescription from there. You will be unable to order or collect your prescription from the surgery.
Test Results – Please call between 2pm and 6pm.
